There are options for Puppet Warp that affect how it works. If this is not the case for you, use the keyboard shortcut, Command (Control) I to invert the mask, or choose Image / Adjustments / Invert from the Main Menu Bar. The Layer thumbnail should show the body in white and the background in black. You can use it, however, if you inverse the selection and expand it by 5-20 pixels prior to adding the Layer Mask.
